If this happens in Software rendering, chances are that you have missing textures. There are times when your map editor won't catch a texture that's missing because the terrain is sometimes modified while in the level, e.g., raised ground becomes a pit.
If this happens in OpenGL rendering, it's almost always the nodebuilder's fault. Drag a vertex near the affected area; that might help. Sometimes, changing the ceiling/floor heights might help.
